Appointments are available within 48 to 72 hours.

HOW DO I MAKE AN APPOINTMENT?

You can make an appointment online via our ‘book an appointment’ button or you can call us at the number listed above and we can book you with the location of your choice.

DOES THE DOCTOR SPEAK SPANISH?

Yes.

DOES THE DOCTOR SEE CHILDREN?

Only children over the age of 15.

WHAT INSURANCES DO YOU ACCEPT?

DO I NEED A REFERRAL?

The following insurances require an authorization or referral in order for you to see the doctor:

  • Neighborhood Health Partnership
  • Simply Healthcare
  • Bluecross Blueshield HMO (Some plans may not require.)
  • Humana Medicare
  • Coventry Summit (Some plans may not require.)

HOW LONG WILL MY VISIT TAKE?

For new patients please allow up to 30 minutes prior to seeing the doctor for paperwork and insurance processing. If we have not received necessary referrals or medical records, additional time may be required prior to seeing the doctor. Dr. Martinez’ goal is to give each patient the time he or she needs for the diagnosis and symptoms presented – no patient is rushed out of the office.

WHAT SHOULD I BRING TO MY FIRST VISIT?

It is essential that the following items are brought to your first visit:

  • Photo identification
  • Insurance card
  • A list of medications you are taking
  • Medical records and imaging studies pertaining to your diagnosis and/or symptoms
